4. Information reasonably sufficient to permit us to contact you, such as your address, telephone number, and email address;
5. A statement by you that you have a good faith belief that use of the disputed material is not authorized by the copyright or other proprietary right owner, its agent, or the law;
6. Identification of the intellectual property that you claim has been infringed by the Site (e.g., "XYZ Copyright," "ABC Trademark, Registration No. 123456, Registration Date 1/1/04," etc.); and
7. A statement by you, made under penalty of perjury, that the above information and notification are accurate and that you are the copyright owner or authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
